Workforce Solutions, 1717 Taylor Ave. - Mondays 1-7pm - Full-service and Homestead only appointments available – North Entrance, check-in at Classroom C
Racine Public Library, 75 7th St. – Tuesdays 10am-2pm (except last Tuesday of each month is 4-7pm) and Thursdays 2-7pm (2-4pm is Spanish service specific) – Full-service and Homestead only appointments available – Free parking in lot, site held on second floor in community room
Burlington Public Library, 166 E. Jefferson St. – Tuesdays noon-3pm – Drop-off appointments available, services provided in lower level, elevator available
RUSD Pathways Campus, 2333 Northwestern Ave. - Wednesdays 1-7pm – Dropp-off, full-service and Homestead only appointments available – parking available in back of building, site held on second floor, elevator available
George Bray YMCA, 924 Center St. – Fridays 9am-noon – Drop-off appointments available
Case High School, 1345 Washington Ave. – Saturdays 9am-1pm – Full-service appointments available – Theatre entrance, site held in the library
What should I bring with me?
Failure to bring all required documentation with you to your appointment will delay your assistance.
We require the documents listed below:
- Picture ID for you and your spouse (if married). Excepted documents include valid driver's license, government issued state ID, school ID or passport.
- Social Security cards for you and your dependents, as well as your spouse if married. We require the actual card (photocopies and digital pictures are acceptable; SSA-1099 statements may be used if you receive Social Security). If you don't have a Social Security number, you will need an Individual Taxpayer Identification Number (ITIN), or the proper proof of foreign status.
- All of your income statements, for you (and your spouse if married).
- W-2 (employment)
- W-2 G (gambling winnings)
- 1099-R (retirement)
- 1099-G (unemployment)
- SSA-1099 (social security)
- 1099-INT (interest)
- 1099-DIV (dividends)
- 1098-T (tuition)
- 1095-A if you receive health insurance through the marketplace.
- Total amount paid for childcare or elder care. You will need the tax ID numbers for those providing the care services. You might be eligible for a dependent care credit.
- Identity Protection PIN (IPPIN) verification letter, if you, your spouse, or any dependents have requested or been assigned one by the IRS.
- If you want your money deposited directly into your bank account, you will need to bring your bank information. A blank or voided check can be used to provide your bank routing number, as well as your account number.
- Understand, too, that if you are married filing jointly, you can file electronically only if you and your spouse are both present to sign the forms that allow for it.
- If filing homestead, you will need to have a completed, unaltered and signed completed rent certificate or property tax bill from the year you are claiming the credit and documentation for all forms of income received including SSI, SSDI, supplemental social security and/or child support received.
